New Client Paperwork

In order to prepare for our first session, it is important that new clients download and complete the following paperwork. Please review the following documents: Services Agreement, Notice of Privacy Practices, and Consumer Rights (Medicaid only). Print and complete the Client Data Sheet, sign the Signature Page and Authorization Form (if applicable), and bring them to the appointment with you. You may also sign them electronically using Docusign.
